In 1992, relatively stable patients at the VA hospital in Columbia, Missouri were dying on one ward’s overnight shift at an alarming rate. A review of the charts showed nurse Richard Williams had been present for most of the Code Blues, but he denied being responsible for the deaths. Whistleblowers accused VA administrators of keeping the situation quiet and retaliating against staff who tried to stop it.
